Report on Logical Disk Free Space % for a group of servers

[MOMBoy]

I need to report on Logical Disk Free Space % for a group of servers. This
group is explicitly populated with SQL server FQDN instance names.

I want the report to be generated only for the drives that have less than 5%
of space left. I tried canned Performance reports, but that doesn't really do
the trick. Amyone has any insites on how to generate this?
