Need to Create Maintenance Mode Report

Does anyone know how to create a report that shows a detailed listing of each
time a server is put into maintenance mode? My Manager would like to see how
many times a server is placed into maintenance mode on a monthly basis. It
would be nice to actually see the comment field for each maintenance mode
occurrance.

Any assistance would be appreciated!
